Job summary

Cedar Point Care & Rehabilitation is seeking a dedicated Physical Therapist to provide rehabilitative therapy in a skilled nursing setting, focusing on cognitive and ADL function recovery and progress documentation. You will work under the direction of the Director of Rehabilitation Services and contribute to a compassionate care environment.

Active state licensure is required, with 2+ years in a skilled nursing facility.

Qualifications

  • Bachelor's degree or higher in Physical Therapy or related field.
  • Active state license.
  • 2+ years of experience in a skilled nursing facility environment.

Responsibilities

  • Screen/evaluate patients with cognitive and/or ADL disorders and develop an appropriate plan of care following regulatory and clinical standards.
  • Utilize standardized assessments and evidence-based practice to support clinical interventions.
  • Provide rehabilitative, skilled and medically necessary treatment interventions per regulatory and clinical requirements.
  • Administrate modalities, ADL retraining, therapeutic activities and exercises as part of the rehabilitation plan.
  • Document treatment minutes and progress in the electronic record per regulatory requirements.
  • Communicate progress and plans with supervisor and health team members.
  • Provide discharge summaries and caregiver counseling as needed.
  • Supervise and support physical therapist assistants, aides and/or students.

Education

Bachelor's degree or higher in Physical Therapy or related field

Work Schedule: Full-Time / Part-Time / PRN
Pay Rate: dependent on experience
JOB SUMMARY

The Physical Therapist is responsible for providing rehabilitative therapy to patients upon referral for cognitive and/or ADL physical functioning disorders following all regulatory and clinical practice requirements. This includes recovery, improvement in function and when possible, a return to a prior level of functioning. Clinical documentation will describe objective measures and justification for continued or discontinued services.

This position reports to the Director of Rehabilitation Services.

JOB DUTIES
  • Effectively screens/evaluates patients and/ with cognitive and/or ADL physical functioning disorders and develops appropriate plan of care following all regulatory and clinical practice standards.
  • Utilizes standardized assessments and evidence-based practice to support clinical interventions.
  • Provides rehabilitative, skilled and medically necessary treatment intervention to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.
  • Administers various procedures as part of the rehabilitation plan including but not limited to: ADL retraining; therapeutic activities; therapeutic exercises; modalities; use of supportive and assistive devices; etc.
  • Assesses patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
  • Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
  • Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the patients’ physical disorders.
  • Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
  • Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.
  • Documents all regulatory and clinically required information in the electronic documentation system and maintains patient records according to company policy.
  • Accurately records treatment minutes for services provided per all regulatory and clinical requirements.
  • Participation in required meetings including but not limited to: patient care conferences, utilization review meetings, rehabilitation meetings, etc.
  • Participate in facility and company required trainings, in-services and conferences.
  • Able to supervise and support physical therapist assistants; rehabilitation aides and/or students.
  • Adherence to State Required practice acts, code of conduct, ethics during professional practice as an Physical Therapist.
  • Adherence to Company requires policy, procedures and code of conduct.
  • Active State Licensure
