CareOne in Concord, MA is seeking a Full-Time Physical Therapist to join our in-house rehab team. You will provide resident rehabilitation care at the highest level, collaborating daily with nursing and clinical staff to achieve stronger patient outcomes.

Qualifications

• Master's or Doctorate in Physical Therapy.
• Current and active Physical Therapist license in the operating state.
• Prefer experience/knowledge in older adult rehabilitation.
• Prefer knowledge of governmental and regulatory guidelines in healthcare settings.

Responsibilities

• Provide resident rehabilitation care within accepted practice methods meeting standards.
• Evaluate residents and establish effective treatment plans.
• Coordinate supervision of treatment plans with staff across disciplines.
• Screen residents according to schedules and practices.
• Provide feedback to supervisor on clinical and rehab issues.
• Document clinical activities to meet requirements.
• Maintain billing records and participate in in-service training.
• May travel to other facilities to provide services.

The Physical Therapist's (PT) overall responsibility is to provide care to the resident at the highest level.

Position Information: We are looking for a Full-Time Physical Therapist to join our team!

Salary Range: $40 - $57 per hour

Compensation will be based on, but not limited to, experience, qualifications, credentials, employment status, and work schedule.

Facility: CareOne at Concord

Practice Setting: Short Term Rehab, Skilled Nursing, and Long-Term Care

Essential Duties and Responsibilities

• Provides and directs resident/resident rehabilitation care within all general accepted practice methods, meeting all center, state, federal, and accrediting standards.
• Evaluates residents/resident needs and establishes effective and efficient treatment plans for the resident/residents assigned.
• Manages and coordinates the supervision of treatment plans with all staff and other disciplines as indicated.
• Participates in the screening of all residents according to directed schedules and practices.
• Provides feedback to the designated supervisor on all clinical and rehabilitation issues.
• Participates in all center directed processes and services.
• Performs necessary clinical documentation to meet center, state, federal, and accrediting agency requirements.
• Maintains complete, accurate and timely billing records to meet center, state, federal, and accrediting agency requirements
• Participates in in-service training programs as requested
• May travel to other facilities to provide services on a limited basis

Position Requirements

• Master's or Doctorate Degree in Physical Therapy.
• Current and Active Physical Therapist license within the state of operations.
• Prefer-experience/knowledge in older adult rehabilitation.
• Prefer knowledge in governmental and regulatory guidelines in designated healthcare settings.
