Job summary

Gurwin Jewish Nursing & Rehabilitation Center, part of Gurwin Healthcare System on a 36-acre campus in Commack, NY, seeks a full-time Physical Therapist to join our dedicated team serving residents with advanced rehabilitative care.

The ideal candidate will hold NY licensure, have at least one year of PT experience, and be able to plan and implement therapy programs, collaborate with families and staff, and participate in ongoing education to promote quality care.

Qualifications

• Current New York State Physical Therapy License required.
• Graduate from a physical therapy program approved by the American Physical Therapy Association or the American Medical Association.
• A minimum of one year experience as a physical therapist.
• Ability to make clinical and managerial decisions within the scope of professional practice to perform physical therapy procedures competently and independently.

Responsibilities

• Reports the patient's initial status, therapy plans and goals, on-going status and discharge plans.
• Provides assessments, plans and goals.
• Instructs patients, families and team members regarding programs that enhance and/or facilitate care and follow-up.
• Complies with all regulatory criteria.
• Provides professional leadership and clinical guidance and expertise to team members.
• Attends regular continuing education programs.
• Adheres to all aspects of Customer Service, including empathy, courteous communication, and respect.

Set on a 36-acre campus in Commack, NY, the Gurwin Healthcare System includes the 460-bed Gurwin Jewish Nursing & Rehabilitation Center, providing short-term rehabilitation and long-term skilled nursing care, memory care, respiratory and ventilator care, palliative and hospice care, social and medical adult day programs, and on-site dialysis and infusion therapy. Sharing the campus is Gurwin's Assisted Living community (Gurwin Jewish ~ Fay J. EDUCATION AND EXPERIENCE REQUIREMENTS
