Gurwin Healthcare System is seeking a full-time Physical Therapist for our Certified Home Care Division. The role covers Nassau and Western Suffolk and surrounding towns, requires NY PT licensure, and at least 1 year of PT experience. You will assess patients, create plans, and guide families and staff to achieve optimal outcomes.

Looking to cover Nassau and Western Suffolk and other towns as needed.

EDUCATION AND EXPERIENCE REQUIREMENTS

• Current New York State Physical Therapy License required.
• Graduate from a physical therapy program approved by the American Physical Therapy Association or the American Medical Association.
• A minimum of one year experience as a physical therapist.
• Ability to make clinical and managerial decisions within the scope of professional practice to perform physical therapy procedures competently and independently.

JOB RESPONSIBILITIES

• Visit Patients in their homes
• Reports the patient's initial status, therapy plans and goals, on-going status and discharge plans.
• Provides assessments, plans and goals.
• Instructs patients, families and team members regarding programs that enhance and/or facilitate care and follow-up.
