Job summary

Gurwin Healthcare System is seeking a full-time Physical Therapist to join our 36-acre Commack, NY campus. We provide a full continuum of care including skilled nursing, rehab, memory care, and home health services.

The role requires a NY license, a PT degree from an APTA/AMA-approved program, and at least one year of clinical experience. You will assess patients, develop plans, and guide families and staff in best practices while advancing your professional skills.

Qualifications

• New York State Physical Therapy License is required.
• Graduate from an APTA/AMA-approved physical therapy program.
• Minimum of one year experience as a physical therapist.
• Ability to make clinical and managerial decisions within professional scope.

Responsibilities

• Reports patient status, therapy plans and discharge plans.
• Provides assessments, plans and goals for therapy.
• Instructs patients, families and team members on programs to enhance care.
• Complies with regulatory criteria in therapy practice.
• Provides professional leadership and clinical guidance to team members.
• Attends regular continuing education programs.
• Demonstrates empathy and courteous communication in line with customer service.

Set on a 36-acre campus in Commack, NY, the Gurwin Healthcare System includes the 460-bed Gurwin Jewish Nursing & Rehabilitation Center, providing short-term rehabilitation and long-term skilled nursing care, memory care, respiratory and ventilator care, palliative and hospice care, social and medical adult day programs, and on-site dialysis and infusion therapy. Sharing the campus is Gurwin’s Assisted Living community (Gurwin Jewish ~ Fay J. EDUCATION AND EXPERIENCE REQUIREMENTS

• Current New York State Physical Therapy License required.
• Graduate from a physical therapy program approved by the American Physical Therapy Association or the American Medical Association.
• A minimum of one year experience as a physical therapist.
• Ability to make clinical and managerial decisions within the scope of professional practice to perform physical therapy procedures competently and independently.

JOB RESPONSIBILITIES

• Reports the patient's initial status, therapy plans and goals, on-going status and discharge plans.
