Job summary

Gurwin Home Care Agency, Commack, NY, seeks a full-time Physical Therapist for our Certified Home Care Division to deliver in-home therapy across Nassau and Western Suffolk. You will assess patients, develop and implement therapy plans, and guide families and team members to support ongoing care and discharge planning.

Requirements include a NYS PT license, PT degree from an approved program, and a minimum of one year of PT experience. Collaborative, patient-centered care is essential.

Qualifications

• Current New York State Physical Therapy License required.
• Graduate from a physical therapy program approved by the APTA or AMA.
• Minimum of one year of experience as a physical therapist.
• Ability to make clinical and managerial decisions within the scope of professional practice.

Responsibilities

• Visit Patients in their homes.
• Reports the patient's initial status, therapy plans and goals, on-going status and discharge plans.
• Provides assessments, plans and goals.
• Instructs patients, families and team members regarding programs that enhance and/or facilitate care and follow-up.
• Complies with all regulatory criteria.
• Provides professional leadership and clinical guidance and expertise to team members.
• Attends regular continuing education programs.
• Adheres to all aspects of Customer Service, including empathy, courteous communication, and respect.

Set on a 36-acre campus in Commack, NY, the Gurwin Healthcare System includes the 460-bed Gurwin Jewish Nursing & Rehabilitation Center, providing short-term rehabilitation and long-term skilled nursing care, memory care, respiratory and ventilator care, palliative and hospice care, social and medical adult day programs, and on-site dialysis and infusion therapy. Looking to cover Nassau and Western Suffolk and other towns as needed.

EDUCATION AND EXPERIENCE REQUIREMENTS

• Current New York State Physical Therapy License required.
• Graduate from a physical therapy program approved by the American Physical Therapy Association or the American Medical Association.
• A minimum of one year experience as a physical therapist.
• Ability to make clinical and managerial decisions within the scope of professional practice to perform physical therapy procedures competently and independently.
