Job summary

Job Details

Job Location: Westerville, OH
Salary Range: $80,000 – $110,000 per year
Sign‑On Bonus: $10,000 (paid in installments)
Job Category: Physical Therapist

Full‑Time: Monday - Friday, Standard Business Hours.
Coverage Location: Westerville, OH & surrounding areas (New Albany, Whitehall, Gahanna, North Linden).
Travel: Required with mileage reimbursement.
Work setting: On‑road patient visits, training at local office or remotely.

Responsibilities
  • Provide physical therapy services to patients according to a written physician's plan of care and as defined in the state Physical Therapy Practice Act.
  • Assess and evaluate therapeutic, rehabilitative/functional status.
  • Develop, implement and direct physical therapy programs that focus on rehabilitation.
  • Evaluate the patient's home environment.
  • Assess patients for muscle strength, mobility, gait, ROM potential for rehab.
  • Direct patients and families/caregivers in the use and care of therapeutic appliances.
  • Determine priority needs for physical therapy.
  • Report the patient's reaction to treatment or changes in their condition to their physician.
  • Initiate physical therapy program and instruct other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient.
  • Instruct other personnel and/or family/caregiver the goals of the physical therapy program for the patient by participating in case conferences.
  • May train the patient in the use of prosthetic devices.
  • Identify patient and family/caregiver needs for other home health services and refer as necessary.
  • Prepare and submit clinical and progress summaries based on the attainment of goals.
  • Participate in discharge planning for the patient.
  • Prepare and submit a clinical progress summary based on the attainment of goals as directed by Organization policy.
  • Provide physical therapy consultation to home families/caregivers when indicated.
  • Provide in‑service education programs for nursing organization personnel as needed.
  • Participate in the peer consultation process.
  • Supervise PTAs according to organization policy and state regulations.
  • When therapy is the only skilled service, instruct, supervise and evaluate home health aide care every two (2) weeks.
  • Collaborate with other team personnel to achieve well‑rounded care.
Requirements
  • Active physical therapy license in the state of Ohio.
  • Bachelor's, master's or doctorate in physical therapy from an accredited program, required.
  • Possess and maintain a current CPR certification.
  • Recent TB test.
  • Valid driver's license and reliable transportation with active automobile insurance.
Qualifications

  • Two years of appropriate experience as a physical therapist preferred.
