Job summary

Total Care Therapy is seeking a compassionate Physical Therapist in Toledo to join our growing team. You will evaluate, develop, and implement individualized treatment plans that help residents improve mobility, restore function, reduce pain, and maintain independence.

You will supervise PTAs, educate residents and staff, and collaborate with an interdisciplinary team to deliver patient-centered care. A state license and CPR/BLS are required; driving is needed for home health settings.

Qualifications

• Current State PT License required.
• CPR/BLS certification current.
• Valid driver’s license with reliable transportation.
• Experience in geriatrics/home health/assisted living or skilled nursing preferred but not required.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments including therapeutic exercise, gait, balance, transfer, and functional mobility training.
• Reassess progress and modify treatment plans as needed.
• Supervise PTAs in accordance with state regulations.
• Complete timely documentation using EMR.
• Educate residents, caregivers, and staff on treatment plans and home exercise programs.
• Collaborate with interdisciplinary team for coordinated patient-centered care.
• Maintain a productive clinical caseload while delivering quality therapy services.

About the Role

We are seeking a compassionate and motivated Physical Therapist to join our growing team. In this role, you will evaluate, develop, and implement individualized treatment plans that help residents improve mobility, restore function, reduce pain, and maintain independence.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments, including therapeutic exercise, gait training, balance training, transfer training, and functional mobility interventions.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the electronic medical record (EMR).
• Educate residents, caregivers, and facility staff on treatment plans, home exercise programs, fall prevention, and safe mobility techniques.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
• Maintain a productive clinical caseload while delivering exceptional therapy services.

Qualifications

• Current State Physical Therapist (PT) License
• Current CPR/BLS Certification
• Valid driver's license, reliable transportation, and auto insurance
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required
