Responsibilities

The Physical Therapist (PT, DPT, RPT) in Home Health assesses and evaluates patient care needs related to functional status, mobility, endurance, stamina, pain, and other medical conditions. Based on this assessment, the therapist develops a treatment plan, performs interventions to improve and enhance patient well-being, and evaluates progress.

• Evaluate the patient's functional status and physical therapy needs, assisting the physician in developing the therapy plan of care.
• Observe and report the patient's response to treatment and any changes in condition to the supervising nurse and/or physician.
• Treat patients to relieve pain, restore function, and maintain maximum functioning according to accepted standards of practice.
• Instruct and inform healthcare team members, patients, and families/caregivers about the plan of care, functional limitations, and progress toward goals.

Education and Experience

Licensure Requirements

• Current Physical Therapy licensure in the state of practice
• Current CPR certification
• Valid driver's license, vehicle insurance, and access to reliable transportation
