We are seeking a skilled and qualified Primary Care Physical Therapist to service our patients throughout the Midlands East area. If you have a passion for providing patient care, improving outcomes, and building community relationships, Your Health Org is the perfect opportunity for you.
Physical Therapist role performs visits in homes, ALF's, and ILF's, clinics, and via Telehealth. This is a salary-based position, with the opportunity to receive a monthly bonus. This is a full-time 12-hour position working a Track Schedule, 7a-7p, 7 days on, 7 days off.

What Are We Looking For?
Your Health is currently seeking a Physical Therapist to join our growing primary care team. Candidates should be compassionate, empathetic, and committed to providing the highest standards of care. Excellent communication and interpersonal skills are essential, as well as the ability to interact positively and motivate patients.
The Physical Therapist will obtain complete health and psychosocial histories, perform physical assessments, provide health education and counseling, and coordinate referrals to other healthcare professionals and community resources. They must use appropriate reporting mechanisms, respect patient confidentiality, and review medical diagnoses, prognosis, procedures, and clinical courses. Community education and outreach are also part of the role.
