Position

We are seeking a dedicated, licensed Physical Therapist to join our team and provide expert rehabilitation services to our residents.

Job Description

You will work with a diverse population, creating and implementing individualized treatment plans to enhance mobility, reduce pain, and improve quality of life. Collaborate with healthcare professionals to provide comprehensive care, promoting independence and functional improvement.

Key Responsibilities

• Assess, develop, and implement individualized physical therapy treatment plans.
• Conduct evaluations and re-evaluations to track progress.
• Provide hands-on therapy, including exercises, manual therapy, and mobility training.
• Document all patient interactions and treatments.
• Educate patients and families on exercises and techniques.
• Collaborate with healthcare team members to coordinate care.
• Ensure a safe environment and monitor patient responses.
• Maintain accurate documentation in medical records.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current, valid Physical Therapy license in New York.
• Experience in long-term care or geriatrics preferred but not required.
• Strong communication and interpersonal skills.
• Compassionate, patient-focused attitude.
• Ability to work effectively with elderly patients and diverse conditions.
