Join to apply for the Physical Therapist role at Fishkill Center for Rehabilitation and Nursing.

Fishkill Center for Rehabilitation and Nursing is seeking a Weekend per diem Physical Therapist (PT) to join our growing Rehab team. The PT provides skilled physical therapy services in our facility consistent with professional standards and policies.

The PT in our facility examines Residents upon admission, then develops and administers therapy plans to manage their conditions, prevent further disabilities, and achieve long-term health benefits.

Responsibilities:

1. Develops an overall therapy plan of care, including treatment and medical diagnosis.
2. Plans duration and frequency of therapy sessions.
3. Uses standardized assessments to validate and monitor the need for skilled therapy services and patient progress.
4. Completes documentation of observations, responses, treatment programs, progress, and notable changes within required timeframes.
5. Handles billing on the date of treatment.
6. Coordinates patient care with other healthcare team members.
7. Supervises physical therapy assistants according to regulations and standards.
8. Assists residents in improving mobility and relieving pain through exercises, hands-on therapy, and devices.
9. Communicates progress and recovery stages to residents.

Qualifications:

• Graduate of an accredited Physical Therapy Program.
• Current New York State PT License.
• Experience in skilled nursing or long-term care preferred.
