Overview:

Assess, plan, organize, and participate in rehabilitative programs that improve functional mobility, relieve pain, and improve or correct disabling conditions resulting from disease or injury.

Provide age-appropriate education for clients and caregivers.

Supervise and assume responsibility for care provided by identified team members.

Education:

  • Bachelors Degree (Required)
  • Masters Degree (Preferred)

Certification/Licensure:

  • Physical Therapist License (Required)

Experience:

  • Perform modalities/therapeutic interventions in accordance with accepted standards of care and department policies and procedures.
  • Provide effective instruction to the patient and family through standardized educational programs and/or individualized training. Initiate, develop, and maintain collaborative interactions with internal and external team members, patients, and family members to improve patient outcomes.
  • Be productive and efficient in daily operations.
  • Select and perform appropriate assessment procedures.
  • Establish and update formal plan of care to address individual patient needs.
