Job Details

The outpatient rehabilitation therapist evaluates and provides therapeutic interventions for individuals with illnesses or injuries affecting mobility, daily activities, cognition, and vocation.

• Responds promptly to physician’s treatment orders.
• Performs thorough patient evaluations and assesses needs.
• Establishes measurable short and long-term goals with patients.
• Develops individualized care plans and communicates with patients and families.
• Applies knowledge of human development to deliver age-appropriate care.
• Progresses patients to reach their highest potential before discharge.
• Maintains safety awareness during treatments and guides staff accordingly.
• Adheres to patient precautions and contraindications.
• Utilizes therapeutic techniques effectively.
• Participates in team and family conferences.
• Coordinates care with physicians and healthcare providers.
• Continuously assesses and modifies treatment plans as needed.
• Meets productivity standards regularly.
• Supervises students, PTA’s, and aides to ensure quality care.
• Completes initial evaluations within 24 hours, accurately and thoroughly.
• Documents patient progress and education consistently and clearly.
• Prepares patients and families for discharge, including education and equipment recommendations.

Requirements

Education: Minimum Bachelor of Science in Physical Therapy; DPT preferred.

Licensure/Certification: NY State license and registration or valid temporary license; BLS certification required by end of probation.

Skills and Abilities: Ability to evaluate and treat a range of conditions, work in a fast-paced environment, document accurately in EPIC, and communicate effectively.

Experience: Outpatient experience preferred but not required.

Salary Range

USD $43.00 - USD $61.00/hr. Compensation depends on qualifications, skills, and experience. Benefits, bonuses, and incentives are additional.
