Job summary

H2 Health Prime Living in Carbondale, CO is seeking a Physical Therapist to provide 1:1 care to residents within a senior living community. You will work in a supportive, clinician-led environment and develop personalized treatment plans.

New graduates are welcome. Outpatient experience is a plus, but passion for patient care matters. Compensation ranges from $48 to $58 per hour with a full benefits package and a flexible schedule, no weekends.

Qualifications

• Must have a degree from an accredited Physical Therapy program.
• Active or eligible state license to practice as a Physical Therapist (PT).
• New graduates are welcome; outpatient experience is a plus, but passion for patient care matters.

Responsibilities

• Evaluate and treat patients with orthopedic, post-operative, neurological, and sports-related conditions in an outpatient setting.
• Design and implement personalized treatment plans aimed at restoring function and enhancing quality of life.
• Document patient progress accurately using EMR.
• Collaborate with a multidisciplinary team to deliver holistic, patient-centered care.
• Contribute to a positive, clean, and professional clinic environment reflecting H2 Health’s culture of excellence.

Compensation & Benefits:

Pay Range: $48.00 - 58.00 per hour, depending on experience and credentials.
