Job summary

Heritage Manor Care is seeking a Physical Therapist to provide high-quality post-acute care. You will screen, evaluate and treat patients, develop plans, set goals, educate families and document outcomes to ensure the best patient results.

The role includes supervising therapist assistants, coordinating treatment plans with reimbursement requirements, and maintaining accurate notes and reports. A BS in Physical Therapy, state licensure, and CPR certification are required.

Qualifications

• Bachelor of Science in Physical Therapy.
• Licensed as required by State.
• CPR certification
