Job summary

Blue Stone Therapy seeks a Physical Therapist (PT) to provide rehabilitation services to adult and geriatric patients at United Living Community in Brookings, SD. The PT will evaluate patients, develop care plans, and implement treatment programs to improve gait, balance, mobility, strength, and independence.

Responsibilities include supervising therapy staff, coordinating with facility teams, maintaining schedules, documenting care, and supporting quality outcomes across multiple partner

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license (or eligibility for licensure) in the applicable state.
• Experience in skilled nursing, long-term care, or post-acute rehabilitation preferred.
• Strong clinical reasoning, communication, and organizational skills.
• Commitment to patient-centered care and professional standards.

• Provide skilled physical therapy treatment focused on functional and measurable outcomes.
• Address gait training, balance, mobility, strength, range of motion, pain, and discharge planning.

• Maintain appropriate patient schedules per established guidelines.
• Complete timely, accurate documentation to support quality care and reimbursement requirements.
• Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes.

Physical Therapist (PT) - Brookings, SD

Blue Stone Therapy is seeking a Physical Therapist (PT) to provide rehabilitation services to adult and geriatric patients at United Living Community in Brookings, SD.

This role focuses on evaluating and treating adult and geriatric patients in a skilled nursing and post-acute rehabilitation setting. The Physical Therapist is responsible for patient assessments, care plan development, and implementation of treatment programs designed to improve gait, balance, mobility, strength, range of motion, pain management, and overall functional independence.

This position will serve multiple partner communities, including Bethel Lutheran in Madison, with travel between locations as needed.
