Job summary

Total Care Therapy is seeking a compassionate Physical Therapist to join our growing team in Brighton, MI. You will evaluate residents, develop and implement individualized treatment plans to improve mobility, reduce pain, and maintain independence in assisted living and skilled settings.

Responsibilities include delivering skilled PT services, supervising PTAs as appropriate, documenting in the EMR, and collaborating with an interdisciplinary team to provide patient-centered care for older

Qualifications

• Current State Physical Therapist (PT) License.
• Current CPR/BLS Certification.
• Valid driver's license, reliable transportation, and auto insurance.
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments, including therapeutic exercise, gait training, balance training, transfer training, and functional mobility interventions.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the electronic medical record (EMR).
• Educate residents, caregivers, and facility staff on treatment plans, home exercise programs, fall prevention, and safe mobility techniques.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
• Maintain a productive clinical caseload while delivering exceptional therapy services.
