Prava-Therapy is seeking a passionate Physical Therapist for our outpatient team in Bloomingdale. The role focuses on patient evaluations with potential to transition into a full-time position, collaborating closely with our COTA.

Qualifications

Responsibilities

• Conduct thorough assessments of patients' physical, cognitive, and emotional needs.
• Develop and implement tailored care plans for ADLs, mobility, and functional skills.
• Collaborate with PT, PTA, and COTA on staff.
• Manage discharge planning by evaluating patient progress.
• Provide case management services for patients in the outpatient clinic.
• Educate patients and caregivers on adaptive techniques and equipment.
• Maintain detailed documentation of patient progress and plans.

Duties

• Conduct thorough assessments of patients’ physical, cognitive, and emotional needs using physiology knowledge and medical terminology
• Develop and implement tailored care plans that address individual goals for activities of daily living (ADLs), mobility, sensory processing, and functional skills
• Collaborate with PT, PTA, and COTA on staff.
• Manage discharge planning by evaluating patient progress
• Provide case management services for patients in our outpatient clinic
• Educate patients and caregivers on adaptive techniques and equipment to support ongoing therapy goals
• Maintain detailed documentation of patient progress, treatment sessions, and care plans in compliance with healthcare standards

Requirements

• Valid state licensure as an Physical Therapist with current registration or certification as required by local regulations
• Proven experience working in outpatient clinics
• Knowledge of anatomy and physiology relevant to occupational therapy practices; familiarity with medical terminology essential
• Skilled in developing care plans that incorporate patient-centered goals and discharge strategies
• Strong case management skills with the ability to coordinate services across multiple providers and settings
• Excellent communication skills to effectively collaborate with healthcare teams and educate patients/caregivers
• Pelvic health experience is not required but strongly preferred
