Qualifications

  • Experience with discharge planning for diverse patient populations.
  • Familiarity with EMR systems for documentation and record management.
  • Ability to work with pediatric, geriatric, orthopedic and acute care patients, with health providers.

Responsibilities
  • Conduct comprehensive patient evaluations to determine physical therapy needs, including assessments of physiology, anatomy, and medical history
  • Develop and execute tailored care plans that align with each patient’s specific goals and medical conditions
  • Deliver hands‑on therapy sessions utilizing techniques such as trigger point therapy, orthopedics, geriatrics, pediatrics, and outpatient modalities
  • Monitor patient progress regularly and adjust treatment strategies accordingly to optimize recovery outcomes
  • Collaborate with multidisciplinary teams to coordinate discharge planning, home & community care services, and follow‑up care
  • Educate patients and their families on exercises, injury prevention, and health maintenance strategies
  • Document all patient interactions accurately within electronic medical record (EMR) systems in compliance with healthcare regulations
Requirements
  • Valid licensure or certification as a licensed Physical Therapist in the relevant jurisdiction
  • Proven experience in inpatient hospital settings, outpatient clinics, or home health environments
  • Strong knowledge of physiology, anatomy, medical terminology, and physiotherapy techniques
  • Experience with discharge planning and developing comprehensive care plans for diverse patient populations
  • Familiarity with EMR systems for documentation and patient record management
  • Demonstrated ability to work effectively with pediatric, geriatric, orthopedic, and acute care patients along with other health providers such as chiropractors, massage therapists and acupuncturists
  • Excellent communication skills to educate patients and collaborate with healthcare teams effectively
