Azza HealthCare Agency is seeking a licensed Physical Therapist for Home Health. You will evaluate patients’ functional status, mobility, endurance, and pain to develop personalized treatment plans and collaborate with physicians.

Responsibilities include creating therapy plans, delivering treatments, monitoring responses, educating patients and families, and documenting progress in home settings. The role emphasizes equity, ongoing growth, and community well-being.

Qualifications

• Current Physical Therapy license in the practice state.
• Valid driver’s license, vehicle insurance, and access to reliable transportation.
• Preferred: CPR certification or ability to obtain within 90 days.
