Job summary

Chestnut Health is seeking a Nurse Practitioner to join our physiatry team. This role focuses on rehabilitation medicine, collaborating with therapists, nurses, and other professionals to restore function and independence for patients.

New graduates are encouraged to apply. Eligible to work in the United States, licensure and DEA/CS qualifications preferred. This patient-centered, team-based environment offers growth in physical medicine and rehabilitation.

Qualifications

• Eligible to work in the United States.
• Actively licensed to practice in the state of employment preferred.
• Current Controlled Substance License by start date preferred.
• Current DEA License by start date preferred.
• ANCC or AANP certification preferred.
• Interest in Physiatry / Physical Medicine and Rehabilitation is a plus.
• Experience in orthopedics is a plus.
• Experience in physiatry is a plus.
• Experience in skilled nursing is a plus.
• Certified Nurse Practitioner certification preferred.

Responsibilities

• Collaborate with therapists, nurses, and other professionals to develop rehab treatment plans.
• Conduct patient assessments including histories, exams, and evaluations to set rehab goals.
• Monitor progress and adjust plans to optimize outcomes and independence.
• Participate in multidisciplinary meetings and educational activities to grow skills.
• Document initial evaluations, plans, progress notes, and discharge summaries in EMR.
• Educate patients and families about rehab process, outcomes, and post-discharge care.
