Qualifications

DEA license or willing to obtain DEA licensure.
Nursing home and/or physiatry experience is a plus, but not required.
Comfort with performing peripheral joint injections (knees, shoulders, hips).
Strong communication and collaboration with multidisciplinary teams.
Familiarity with EHR charting.

Responsibilities

Rounding at 2 SNFs, 5 days per week, delivering physiatry care to 25+ patients daily.
Performing focused assessments and developing individualized treatment plans with facility teams to optimize therapy participation and recovery.
Providing interventions such as joint injections, Botox, dry needling, and medication management as needed.
Serving as the sole physiatrist on-site with autonomy, supported by a clinical team for consultation.
Coordinating care and referring to other specialties when appropriate.

We're Hiring:

We are seeking a passionate, motivated provider to join us full-time and independently offer care and consultations at our contracted facilities.

Day-to-Day Responsibilities

As a physiatrist, you will round at 2 Skilled Nursing Facilities, 5 days per week (2 days at one location, 3 days at the other). Your primary focus will be delivering specialized physiatry care to 25+ patients per day who are experiencing difficulty participating in therapy due to pain or functional limitations.

Key Responsibilities Include

Performing focused assessments and developing individualized treatment plans in collaboration with facility primary care teams to optimize therapy participation and functional recovery.
Providing targeted interventions such as joint injections, Botox treatments, dry needling, and medication management based on patient-specific needs.
Serving as the sole physiatrist on-site, operating with a high level of autonomy while supported by a collaborative clinical team for consultation as needed.
Coordinating care and referring to other specialties or services for issues beyond the scope of physiatry.

This role is best suited for a self-motivated and confident clinician who thrives in independent practice environments and is passionate about improving rehabilitation outcomes.

Qualifications

Current, unrestricted Physician Assistant (PA) or Nurse Practitioner (NP) license.
DEA licensed or willing to obtain DEA licensure.
Nursing home and/or physiatry experience is a plus, but not required.
Comfort and interest in performing peripheral joint injections (knees, shoulders, hips).
A genuine passion for providing professional, personalized care in an autonomous environment.
Strong communication skills, with the ability to collaborate with multidisciplinary teams.
Familiarity with EHR charting.
