Iconic Care is seeking a full-time physiatrist to provide on-site care at contracted facilities. You will round at multiple Skilled Nursing Facilities, delivering physiatry services and working with facility teams to improve patient participation in rehabilitation.

Qualifications

Current, unrestricted PA or NP license.
DEA license or willingness to obtain DEA licensure.
Nursing home and/or physiatry experience is a plus, but not required.
Comfort performing peripheral joint injections (knees, shoulders, hips).
Autonomous, professional care with strong communication and collaboration.

Responsibilities

Round at 1-3 SNFs or equivalent centers, 1-3 days per week, caring for 25+ patients daily.
Develop individualized treatment plans with facility teams to improve therapy participation.
Provide interventions such as joint injections, Botox, dry needling, and medication management.
Serve as sole on-site physiatrist with autonomy, supported by a clinical team as needed.
Coordinate care and refer to other specialties when needed.

We're Hiring

We are seeking a passionate, motivated provider to join us full-time and independently offer care and consultations at our contracted facilities.

Day-to-Day Responsibilities

As a physiatrist, you will round at 1-3 Skilled Nursing Facilities or similar rehabilitation centers, 1-3 days per week at each location. Your primary focus will be delivering specialized physiatry care to 25+ patients per day who are experiencing difficulty participating in therapy due to pain or functional limitations.

Key Responsibilities Include

Performing focused assessments and developing individualized treatment plans in collaboration with facility primary care teams to optimize therapy participation and functional recovery.
Providing targeted interventions such as joint injections, Botox treatments, dry needling, and medication management based on patient-specific needs.
Serving as the sole physiatrist on-site, operating with a high level of autonomy while supported by a collaborative clinical team for consultation as needed.
Coordinating care and referring to other specialties or services for issues beyond the scope of physiatry.

This role is best suited for a self-motivated and confident clinician who thrives in independent practice environments and is passionate about improving rehabilitation outcomes.
