Job summary

Iconic Care is seeking a Physiatry Physician Assistant (or Nurse Practitioner) to deliver rehab-focused care at two skilled nursing facilities in Idaho Falls. You will round five days a week (2 days at one site, 3 days at the other) and manage 25+ patients daily, performing assessments, treatment planning, joints injections, and medication management with a collaborative team.

• Familiar with EHR charting.

Physiatry Physician Assistant (Nurse Practitioner)

Day-to-Day Responsibilities

As a physiatrist, you will round at 2 Skilled Nursing Facilities, 5 days per week (2 days at one location, 3 days at the other). Your primary focus will be delivering specialized physiatry care to 25+ patients per day who are experiencing difficulty participating in therapy due to pain or functional limitations.

• Performing focused assessments and developing individualized treatment plans in collaboration with facility primary care teams to optimize therapy participation and functional recovery.
• Providing targeted interventions such as joint injections, Botox treatments, dry needling, and medication management based on patient-specific needs.
• Serving as the sole physiatrist on-site, operating with a high level of autonomy while supported by a collaborative clinical team for consultation as needed.
• Coordinating care and referring to other specialties or services for issues beyond the scope of physiatry.

This role is best suited for a self-motivated and confident clinician who thrives in independent practice environments and is passionate about improving rehabilitation outcomes.

Qualifications:

• Current, unrestricted Physician Assistant (PA) or Nurse Practitioner (NP) license.
• DEA licensed or willing to obtain DEA licensure.
• Nursing home and/or physiatry experience is a plus, but not required.
• Comfort and interest in performing peripheral joint injections (knees, shoulders, hips).

• Strong communication skills, with the ability to collaborate with multidisciplinary teams.
