Job summary

Chestnut Health Systems in Jacksonville, IL, is seeking a dedicated Nurse Practitioner to join our Physiatry team. This role focuses on rehabilitation and improving functional outcomes through comprehensive patient assessments and collaborative care with therapists and nurses.

The successful candidate will be licensed in the United States, ideally with DEA and CS licenses, ANCC or AANP certification, and interest in Physiatry. New graduates are welcome to apply.

Qualifications

• Eligible to work in the United States.
• Actively licensed to practice in the state of employment preferred.
• Current DEA license preferred.
• Current controlled substances license preferred.
• ANCC or AANP certification preferred.
• Interest in working in Physiatry/Physical Medicine and Rehabilitation is a plus.
• Experience in orthopedics/physiatry/skilled nursing is a plus.

Responsibilities

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs.
• Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine rehabilitation goals and interventions.
• Monitor patient progress closely and adjust treatment plans to optimize functional outcomes and independence.
• Participate in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills.
• Maintain accurate and timely documentation of evaluations, treatment plans, progress notes, and discharge summaries in EMR systems.
• Provide education and counseling to patients and families regarding rehabilitation process, outcomes, and post-discharge care.
