Job summary

Chestnut Health Company is seeking a compassionate Nurse Practitioner to join our Physiatry team. The role focuses on restoring function and improving quality of life within a collaborative, patient-centered rehab setting.

The NP will work with therapists and clinicians to implement comprehensive treatment plans, monitor progress, and provide education to patients and families. A strong interest in physiatry and prior related experience are valued, with a commitment to work-life balance.

Qualifications

• Actively licensed to practice in the state of employment.
• Current Controlled Substance License (required upon start date).
• Current DEA License (required upon start date).
• ANCC Certification or AANP Certification.
• Desire to work in Physiatry/Physical Medicine and Rehabilitation.
• Orthopedic Experience (nice to have).
• Physiatry Experience (nice to have).
• Skilled Nursing Experience (nice to have).

Responsibilities

• Collaborate with therapists, nurses, and other healthcare professionals to develop comprehensive treatment plans for rehabilitation patients.
• Conduct thorough patient assessments including histories, exams, and evaluations to set rehabilitation goals.
• Monitor progress and adjust treatment plans to optimize function and independence.
• Participate in multidisciplinary meetings, case conferences, and continuing education.
• Maintain accurate documentation of evaluations, plans, progress notes, and discharges in EMR systems.
• Educate patients and families about the rehabilitation process and post-discharge care.
