Chestnut Health Company in Belleville, IL is seeking a Nurse Practitioner to join our Physiatry team within Physical Medicine and Rehabilitation. This role emphasizes collaborative, patient-centered rehabilitation plans to restore function and independence.

Qualifications

Active licensed to practice in the state of employment.
Current Controlled Substance License.
Current DEA License.
ANCC or AANP certification.
Interest in Physiatry/PM&R.
Orthopedic or Skilled Nursing experience is a plus.

Responsibilities

Collaborate with therapists, nurses, and other professionals to create comprehensive rehab plans.
Perform thorough patient assessments to determine rehabilitation goals and interventions.
Monitor patient progress and adjust plans to optimize functional outcomes and independence.
Participate in multidisciplinary meetings, case conferences, and educational activities to enhance knowledge.
Document initial evaluations, treatment plans, progress notes, and discharge summaries in EMR.
Educate patients and families about the rehabilitation process, outcomes, and post-discharge care.
