Chestnut Health Company in Phoenix, AZ is seeking a Nurse Practitioner to join our Physiatry team, focusing on rehabilitation and improving functional independence. New graduates are encouraged to apply, with mentorship in a collaborative, patient-centered environment.

Responsibilities include developing and implementing rehab treatment plans with therapists and nurses, conducting assessments, documenting in EMR, and educating patients and families about the rehab process.

Qualifications

• Actively licensed to practice in the state of employed.
• Current Controlled Substance License (required upon start date).
• Current DEA License (required upon start date).
• ANCC or AANP certification as Nurse Practitioner.
• Desire to work in Physiatry/PM&R.
• Orthopedic experience welcomed.
• Physiatry experience welcomed.
• Skilled Nursing experience welcomed.

Responsibilities

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive rehabilitation plans.
• Conduct patient assessments, medical histories, exams, and diagnostic evaluations to set rehab goals.
• Monitor progress and adjust treatment plans to optimize functional outcomes and independence.
• Participate in multidisciplinary meetings and educational activities to enhance clinical skills.
• Maintain accurate documentation of evaluations, plans, and discharge summaries in EMR systems.
• Educate patients and families about rehabilitation process, outcomes, and post-discharge care.
