Job summary

Chestnut Health Company is seeking a Nurse Practitioner to join our physiatry team. You will work in a collaborative, patient-centered environment focused on rehabilitation and improving function and independence for patients with injuries or disabilities.

Qualifications

• Active NP license to practice in the state of employment.
• Current Controlled Substance license (required on start).
• Current DEA license (required on start).
• ANCC or AANP certification.
• Interest in Physiatry/PM&R and rehab.
• Orthopedic, Physiatry or skilled nursing experience is a plus.

Responsibilities

• Collaborate with therapists and nurses to develop and implement comprehensive rehab treatment plans.
• Conduct thorough patient assessments including histories and examinations to establish rehab goals.
• Monitor progress and adjust plans to optimize functional outcomes and independence.
• Participate in multidisciplinary meetings and ongoing clinical education.
• Maintain accurate documentation in electronic medical records (evaluation, plans, progress, discharge).
• Educate patients and families about rehab process, outcomes, and post-discharge care.
