Job summary

Chestnut Health in Kentucky is seeking a compassionate Nurse Practitioner to join our physiatry team. You will collaborate with therapists and nursing staff to design and execute comprehensive rehabilitation plans focused on functional improvement and independence.

Qualifications

Active NP license in the state of employment.
Current DEA license required by start date.
Current controlled substances license required by start date.
ANCC or AANP certification.
Certified Nurse Practitioner credential required.
Interest in Physiatry/PM&R and rehab medicine.
Orthopedic experience is a plus.
PM&R experience is a plus.
Skilled Nursing experience is a plus.

Responsibilities

Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive rehabilitation plans.
Conduct thorough patient assessments including medical histories and physical examinations to inform rehab goals.
Monitor progress and adjust treatment plans to optimize functional outcomes and independence.
Participate in multidisciplinary team meetings and educational activities to enhance clinical skills.
Document evaluations, treatment plans, progress notes, and discharge summaries accurately in EMR systems.
Educate patients and families about rehabilitation processes, expected outcomes, and post-discharge care.
