Job summary

Chestnut Health Company is seeking a compassionate Nurse Practitioner to join our physiatry team in a Skilled Nursing Facility setting. This role emphasizes collaborative care, comprehensive rehab planning, and patient-centered outcomes to restore function and independence.

Qualifications

Licensed to practice in the state where employed.
Current Controlled Substance License required on start date.
Current DEA license required on start date.
ANCC or AANP certification required.
Desire to work in physiatry/PM&R.
Orthopedic experience is a plus.
Physiatry experience is a plus.
Skilled Nursing Facility experience is a plus.

Responsibilities

Collaborate with therapists, nurses, and other professionals to develop comprehensive rehab plans.
Conduct patient assessments including histories, exams, and evaluations to determine goals.
Monitor progress and adjust treatment plans to optimize outcomes.
Participate in multidisciplinary meetings and ongoing education activities.
Document initial evaluations, plans, progress notes, and discharge summaries in EMR.
Educate patients and families about rehabilitation, outcomes, and discharge care.
