Job summary

Chestnut Health Company is seeking a Nurse Practitioner to join its Physiatry team in a patient-centered environment focused on functional outcomes. The role emphasizes collaboration with therapists, nurses, and other clinicians to implement comprehensive rehabilitation plans.

The position involves conducting thorough assessments, monitoring progress, and educating patients and families about the rehabilitation process and post-discharge care. Strong teamwork and PM&R interest are valued.

Qualifications

Eligible to work in the United States.
Active license to practice in the state of employment preferred.
Controlled Substance license by start date preferred.
DEA license by start date preferred.
ANCC or AANP certification preferred.
Interest in Physiatry / PM&R is a plus.
Experience in orthopedics is a plus.
Experience in physiatry or skilled nursing is a plus.

Responsibilities

Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs.
Conduct thorough patient assessments including histories and examinations to determine rehabilitation goals.
Monitor progress and adjust treatment plans to optimize functional outcomes.
Participate in multidisciplinary meetings and educational activities to enhance knowledge and skills.
Maintain accurate documentation in electronic medical records.
Educate patients and families about the rehabilitation process and post-discharge care.
