Job summary

Qualifications

• 7+ years in healthcare IT, population health, or related leadership.
• Experience deploying and supporting enterprise health IT in clinical contexts.
• Proven leadership of 7–8 person teams with cross-functional coordination.
• Strong documentation, onboarding, access management, audit readiness.
• ITIL, PMP, Agile, or change-management certifications preferred.
• Ability to translate clinical and technical topics for varied audiences.

Responsibilities

• Identify and lead implementation of population health platforms across practices.
• Partner with provider offices, clinical teams, operations, and exec stakeholders.
• Drive adoption, training, and ongoing support to improve utilization and outcomes.
• Develop measurable strategies for readiness and engagement.
• Manage incident, change, and service processes aligned with ITSM.
