Description:

Supports the Trinity Health Alliance of Michigan pharmacy team to enhance workflow for clinical pharmacy services across the network, including medication therapy management (medication reviews and chronic disease management) and medication adherence. Provides patient identification, outreach, and scheduling support for pharmacists performing clinical services and telephonic patient care (pre-visit medication reconciliation, adherence monitoring). Supports data/metric tracking, trending, and reporting.

The Population Health Pharmacy technician primarily handles outreach and scheduling for medication management services, educates and supports patients with medication adherence. This is a remote position, ideally based in Michigan.

Qualifications:

• Education: Required: High School Diploma/GED; Preferred: Post-secondary education (vocational, technical, diploma, bachelor's)
• Credentials/Licensure: Required: Michigan Board of Pharmacy Technician registration/license; Certified Pharmacy Technician (CPhT) with PTCB. Preferred: Pharmacy technician training program graduate; BLS certification.
• Experience: Required: Three years of pharmacy technician practice
