Altruix is seeking a Pharmacy Technician - Patient Care Coordinator (PCC) to join our Crossroads team at the Uniontown LTC pharmacy, PA. You will coordinate medication needs for patients, work closely with Crossroads facilities, providers, and the Altruix pharmacy team to ensure timely access to medications and an exceptional patient experience.

The role is primarily onsite at 278 McClellandtown Road, Uniontown, PA 15401, with occasional travel to Crossroads facilities to provide onsite support.

Qualifications

• Requires PA Pharmacy Technician registration or Trainee registration.
• Previous experience in a pharmacy, healthcare, or medical office setting.
• Excellent communication and ability to manage multiple priorities.

Responsabilités

• Serve as liaison between Crossroads facilities and Altruix pharmacy team.
• Coordinate medication needs and patient communications.
• Input patient data and prescriptions into pharmacy systems.
• Assist with inventory management and cycle counts.
• Travel to Crossroads facilities as needed.
