Qualifications

• Doctor of Pharmacy degree from an ACPE accredited school.
• Active New York State Pharmacist Licensure.
• PGY1 and PGY2 pharmacy residency training.
• Board certification in Pharmacotherapy or other specialty required within 18 months.

Responsibilities

• Coordinates pharmacotherapy needs for patients in outpatient practice.
• Maintains contact with assigned caseload and initiates appropriate responses.
• Communicates with multidisciplinary team to optimize patient care.
• Provides patient and caregiver medication counseling.
