Candidates must have a Doctor of Pharmacy degree, New York State Pharmacist licensure, and complete relevant residencies. This role involves managing pharmacotherapy needs, conducting clinical interventions, and contributing to medication safety efforts.

The position also offers a competitive salary range of $121,792.21 to $190,000.00 annually, along with a comprehensive benefits package focused on wellness and employee support.

Qualifications

• Doctor of Pharmacy degree from an ACPE accredited school of pharmacy.
• New York State Pharmacist licensure with current NYS registration.
• PGY1 pharmacy residency training.
• PGY2 pharmacy residency training.
• Board certification in Oncology pharmacy (BCOP) within 12 months of hire.
• Collaborative Drug Therapy Management certification within 12 months of hire.

Responsibilities

• Manage a clinical pharmacotherapy program within an oncology team.
• Develop, implement, and manage a CDTM agreement.
• Evaluate and report measures of patient encounter productivity.
• Maintain proficiency in USP Chapter <797> compounding procedures.
• Participate in tumor boards for selected hematology/oncology practice areas.
