Qualifications

  • Doctor of Pharmacy degree from an ACPE accredited school of pharmacy.
  • New York State Pharmacist licensure with current NYS registration.
  • PGY1 pharmacy residency training.
  • PGY2 pharmacy residency training.
  • Board certification in Oncology pharmacy (BCOP) within 12 months of hire.
  • Collaborative Drug Therapy Management certification within 12 months of hire.

Responsibilities

  • Manage a clinical pharmacotherapy program within an oncology team.
  • Develop, implement, and manage a CDTM agreement.
  • Evaluate and report measures of patient encounter productivity.
  • Maintain proficiency in USP Chapter <797> compounding procedures.
  • Participate in tumor boards for selected hematology/oncology practice areas.
