Qualifications

• Active Pharmacy License in the state of employment.
• Immunization Certification from an accredited organization.
• Pharmacist in charge (pharmacist in charge listed on state license).
• Not listed on the DEA Excluded Parties List.
• Submission of required PMP information to register for PMP access where applicable.
