Qualifications

• Active Pharmacy License in the state of employment.
• Not on the DEA Excluded Parties List.
• Immunization Certification through an accredited organization (e.g. APhA).
• Listed as pharmacist in charge on the state license.
• Submission of required PMP information to the state PMP administrator where applicable.

Responsibilities
