Job summary

NALHE seeks a licensed pharmacist to work under supervision, dispensing and preparing medications, consulting with patients and medical staff about therapy, and ensuring accurate documentation and safety. The role supports hospital and ambulatory settings with emphasis on pharmaceutical care and device guidance.

Responsibilities include adherence to policies, patient interviews for medication histories, and collaboration with care teams to optimize treatment outcomes while maintaining quality

Qualifications

• Dispenses, compounds, procures, stores and distributes pharmacy products according to laws, regulations and policies.
• Interviews patients to obtain medication histories and educate on use, storage and compliance.
• Communicates with medical staff to optimize medication therapy.
• Substitutes for pharmacy personnel as needed.

Responsibilities

• Upholds Kaiser Permanente policies, procedures and applicable laws.
• Assesses patient data to determine age-appropriate pharmaceutical care.
• Documents prescriptions and related actions in the pharmacy record.
• Interacts with patients about allergies, interactions and administration of medications.
• Counsels patients and coordinates medication therapy with diet and other therapies.
• Collaborates with medical personnel on drug therapy and potential changes.
• Participates in education, quality management and training programs.
• Maintains a clean, organized and safe workspace and equipment.
