Job summary

Amerita, an infusion-focused provider, is seeking an Intake Coordinator to ensure the timely clearance of new referrals in a Monday–Friday 8:30am–5:00pm setting. The role is based in Salt Lake City, UT, and involves payer communications, data entry, and cross-department collaboration.

You will verify coverage, obtain authorizations, enter patient demographics, and keep patient information confidential while supporting the workflow of pharmacists and clinical teams.

Qualifications

• High School Diploma or GED required; associate degree preferred.
• Experience collecting referral information in healthcare.
• Experience with payer types including Medicare/Medicaid.

Responsibilities

• Takes infusion therapy referrals from referral sources.
• Ensures intake forms are complete and within Amerita’s scope.
• Obtains authorizations and re-authorizations as required by payers.
• Enters patient demographics, insurance and authorization data.
• Communicates with other departments regarding referral status.
• Notifies patients/families regarding coverage and payment responsibilities.
• Verifies insurance contracts and ensures authorization before giving to a Pharmacist.
• Maintains confidentiality of patient information.

Responsibilities

• Takes Infusion Therapy referrals from referral sources.
• Ensures that all intake forms are complete, clear and within Amerita’s scope of service.
• Obtains authorizations and re-authorizations as required by payers.
• Enters patient demographic, insurance and authorization information into the computer system.
• Communicates with other departments regarding the status of referrals.
• Notifies patients/families regarding coverage and payment responsibilities.
• Understands which insurance companies have active contracts with Amerita.
• Ensures that insurance verification is completed and authorization is in place prior to giving the referral to a Pharmacist.
• Maintains confidentiality of patient and proprietary information.

Qualifications

• High School Diploma/GED or equivalent required; Associate’s Degree or some college preferred
• Minimum two (2) years of experience collecting referral information in the healthcare market
• Experience working with all payer types, including Medicare, Medicaid and commercial insurance companies
• Knowledge of insurance verification and pre-certification procedures
• Understands the scope of services that Amerita can provide
• Strong verbal and written communication skills
