• Tracks refills due, requests provider orders, and resolves barriers to care with a pharmacist.
• Researches copay, foundation assistance, drug replacement, and patient financial resources.
• Manages prior authorizations and patient assistance enrollments nearing expiration.
• Performs onboarding, benefits investigation, order entry, and delivery coordination.
• Ensures patients receive resources to maintain adherence to their plan.
