Job summary

Qualifications

• State license to practice.
• Experience with EPIC electronic health records.
• Experience with medication reconciliation for discharge medications.
• Ability to gather home medication lists from patients, families or pharmacies.
• Ability to perform discharge counseling with patients and families.
• Knowledge of antimicrobial stewardship and interpretation of culture results.
• Knowledge of pharmacokinetic dosing management and monitoring.
• Knowledge of anticoagulation dosing management and monitoring.

Responsibilities

• Ensure safe and thoughtful distribution of medications.
• Perform medication reconciliation for discharge medications.
• Provide discharge counseling to patients and families.
• Apply antimicrobial stewardship knowledge in patient care.
• Monitor pharmacokinetic and anticoagulation dosing as needed.

Job description

We're hiring a licensed pharmacist for a contract position where you'll use your clinical expertise to ensure the safe and thoughtful distribution of medications to individuals navigating a range of physical and mental health challenges.

Minimum Requirements:

• Must have State License to Apply
• EPIC experience
• Experience with medication reconciliation for discharge medication lists
• Communication skills for gathering prior to admission home medication lists from patients, families, or pharmacies
• Communication skills for discharge counseling with patients and families
• Clinical knowledge of antimicrobial stewardship, including interpretation of culture results and knowledge of UTI, pneumonia, and SSTI
• Clinical knowledge of pharmacokinetic dosing management and monitoring
• Clinical knowledge of anticoagulation dosing management and monitoring
