  • Monitor for adverse drug events and drug interactions, escalating concerns to the care team as needed.
  • Collaborate with prescribers and other clinicians to optimize therapy plans and address drug shortages or shortages.
  • Document interventions and medication histories in the electronic health record to maintain a complete record of care.

Pharmacist - General

Position Overview

In this general pharmacy role, the Pharmacist ensures safe and effective medication use across diverse patient populations in community or health-system settings. The position emphasizes collaboration with clinicians to optimize therapeutic outcomes and maintain medication safety.


The Pharmacist dispenses medications, counsels patients, and reviews orders for accuracy, drug interactions, and appropriate dosing as part of the care team.

Experience


  • Experience providing medication therapy management and patient counseling in hospital or community settings

  • Experience using electronic health records and pharmacy information systems

  • Experience participating in multidisciplinary rounds or care planning meetings

  • Experience with adverse event reporting and quality improvement initiatives


Preferred


  • Experience in a fast-paced clinical environment such as emergency, ICU, or ambulatory care

  • Immunization program administration experience and strong communication skills

  • Ability to work flexible shifts to support coverage


Work Environment

The role operates in hospital, health-system, or community pharmacy settings with a multidisciplinary team of clinicians. The pace varies with patient volume and clinical demands, and documentation is completed in the electronic health record. Some on-call coverage may be required.


Physical demands include standing for extended periods and handling medications and supplies in a safe, compliant manner.
