Job Description
In this general pharmacy role, the Pharmacist ensures safe and effective medication use across diverse patient populations in community or health-system settings. The position emphasizes collaboration with clinicians to optimize therapeutic outcomes and maintain medication safety.
The Pharmacist dispenses medications, counsels patients, and reviews orders for accuracy, drug interactions, and appropriate dosing as part of the care team.
Education: Doctor of Pharmacy (PharmD) from an ACPE-accredited program.
Licensure: Current pharmacist license to practice in the United States and state-specific licensure as required.
Certifications:
Experience or Skills: Strong medication knowledge, excellent communication, and ability to work in a multidisciplinary team with proficiency in medication safety and pharmacovigilance.
The role operates in hospital, health-system, or community pharmacy settings with a multidisciplinary team of clinicians. The pace varies with patient volume and clinical demands, and documentation is completed in the electronic health record. Some on-call coverage may be required.
