Scope Of Position
Under the direction of the Director of Pharmacy, provides advanced pharmaceutical care by assisting in the development and coordination of clinical pharmacy services and educational programs for the Department. Responsible for providing tools and educational opportunities to enhance services provided by all staff members. Participates on intra- and interdepartmental committees as assigned. Serves as preceptor to pharmacy students and PGY-1 Pharmacy residents as assigned.
Participates in the development of institution-wide drug use policies. Performs a variety of duties involving the receiving, storage, compounding, and dispensing of medications and supplies. Responsible for the assessment of patient drug therapy, review of drug interactions, incompatibilities, drug-induced adverse effects, and appropriate drug doses. Documents the care provided to patients. Participates in supervision of technical staff.
Responsibilities
- Works with staff, physicians, nurses, and other healthcare professionals to develop and monitor medication-use systems that promote safe and effective medication use as part of the interdisciplinary care team.
- Clinically reviews medication orders from house-wide inpatient clinical areas to ensure that the medication as ordered is safe and appropriate, intervening with the clinician on behalf of the patient.
- Interprets medication orders and transcribes/verifies to computerized patient medication profiles accurately.
- Compounds and dispenses pharmaceuticals, including sterile and parenteral nutrition products, accurately for the patient population.
- Issues controlled substances to patient care areas and maintains records as required by law.
- Monitors drug therapy regimens for contraindications, drug interactions, allergies, and appropriateness of drug and dose for the patient population.
- Provides clinical consultation and clarification to practitioners. Suggests appropriate, cost-effective therapeutic alternatives to medical staff, as needed.
- Provides accurate, adequate, and timely drug information to the professional staff.
- Participates in quality improvement and medication use review activities of the department.
- Oversees central pharmacy operations as assigned. Supervises and directs pharmacy support personnel.
- Works independently with minimal supervision. Organizes and prioritizes work assignments. Ensures pharmacy services are provided in a timely manner.
- Performs other related duties as required.
Requirements
- Ability to verbally communicate effectively with multiple levels of personnel/interdisciplinary teams, with tact and confidentiality.
- Ability to work in a fast-paced environment.
- Knowledge of pharmacy information systems; keeping abreast of developments in software and technology.
- Proficient in all aspects of compounding, preparing, and dispensing drugs.
- Working knowledge of federal and state laws related to drug procurement, storage, dispensing, etc.
- Willing to maintain appropriate knowledge to provide state-of-the-art service.
- Ability to maintain a collaborative practice relationship with physicians and other healthcare professionals.
- Ability to meet the needs of patients of all ages.
- Graduate of an accredited school of Pharmacy.
- Three to five years of inpatient pharmacy experience or completed PGY1 residency preferred.
- Current pharmacist license in Connecticut.
- Board certification in a pharmacy specialty within the first 3 years of employment is preferred.
