Job description

The Staff Pharmacist is responsible for dispensing and compounding medications and pharmaceutical supplies upon receiving medication orders from prescribers, in accordance with State and Federal laws and departmental policies and procedures. Due to additional demonstrated clinical expertise, the Pharmacist I serves as a leader in the provision of pharmaceutical care.

Responsibilities

1. Review and interpret medication orders using patient profiles to ensure appropriate pharmaceutical care, including avoiding undesirable drug interactions and duplication of therapy.
2. Prepare and dispense IV parenterals and chemotherapy using aseptic techniques and appropriate compounding equipment in an IV sterile product area.
3. Consult with nurses, physicians, other healthcare professionals, and laypersons to provide drug information at the listener's level. Provide in-services for staff and/or community.
4. Provide medication counseling as required, including for controlled substances.
5. Maintain appropriate inventory and records on controlled substances.
6. Supervise and instruct Pharmacy Technicians and other support personnel, actively offering guidance and oversight.
7. Prepare medications through extemporaneous compounding procedures when necessary.
8. Assist prescribers in making appropriate formulary selections.
9. Perform other related duties with attention to detail and a positive attitude.

Qualifications/Requirements

Experience

• Previous hospital pharmacy practice experience is desirable.

Education

• Bachelor of Science Degree in Pharmacy or PharmD from an accredited college.
• Current license and registration by New York State.

Licenses/Certifications

• NYS Pharmacist License and Registration.

Other

• Potential contact with human blood and other infectious materials; universal precautions, personal protective equipment, and infection control policies must be followed.
• 10 CEU (live), ACPE approved, excellent interpersonal skills, and the ability to supervise support staff effectively.
