Job summary

Valley Baptist Health System in Brownsville, TX seeks a Staff Pharmacist to provide pharmaceutical care, oversee dispensing, and ensure compliance with laws and regulations. The role includes collaboration with physicians and the medical team, medication education, and participation in staff development.

Pharmacists with a Pharm.D. or BS in Pharmacy and an active Texas license are encouraged to apply. Hospital experience is preferred and opportunities may include a sign-on bonus.

Qualifications

  • Pharm.D. or BS in Pharmacy.
  • A valid Texas pharmacy license is required.
  • Experience in hospital setting preferred.
  • If direct patient care responsibilities are involved, CPR/AHA BLS, ACLS, and PALS are required.
  • Board Certification in Pharmacotherapy (BCPS) and CGP are preferred.

Responsibilities

  • Review and monitor patients' medication therapy for safety and efficacy.
  • Verify orders, conduct medication reviews for interactions and dosing.
  • Prepare, label, and dispense medications per prescriptions and regulations.
  • Monitor high-risk IVs, renal dose adjustments, and IV-to-PO conversions.
  • Ensure medication availability, proper storage, and procurement.
  • Supervise preparation, dispensing processes, and staff involved.
  • Maintain relationships with medical and nursing staff; provide drug information.
  • Assist in medication reconciliation and clarify unsafe therapies.
  • Educate patients on medication use and participate in staff education.
  • Demonstrate regulatory compliance through inspections and licensing upkeep.
  • Maintain professional competence through ongoing education.

Job Summary

The Staff Pharmacist provides pharmaceutical care services to patients, including oversight, monitoring, preparation, dispensing, and proper documentation of patient medication therapy, as well as providing drug information, patient information, and oversight of dispensing systems. Additionally, they ensure compliance with Federal, State, and local laws and regulations and may advise physicians on medication utilization. This position may qualify for a sign‑on bonus.

Responsibilities
  • Review and monitor patient’s medication therapy and orders for safety and efficacy, considering specific patient conditions and health status; advise the patient care team and document interventions.
  • Verify orders and perform medication profile reviews, checking for duplications, drug–drug and drug–food interactions, allergies, drug appropriateness, dosing, and frequency.
  • Prepare, label, and dispense medications in accordance with physicians’ prescriptions; ensure compliance with Federal, State, and local laws and the maintenance of required records.
  • Check high‑risk IVs, renal dose–adjusting medications, IV to PO conversions, initiate/modify/discontinue therapy, and mitigate black‑box warning interventions.
  • Ensure appropriate medications are available, properly procured, prepared, stored, and maintained.
  • Supervise the preparation, maintenance, and dispensing process of drugs and supplies, including technical personnel performing these functions.
  • Maintain relationships with medical and nursing staff; provide timely information on pharmaceutical supplies, drug usage and compatibility, state and federal regulations, and Joint Commission standards.
  • Provide collaborative practice management for anticoagulation, parenteral nutrition, vancomycin, and aminoglycoside consults, as per facility‑approved protocols.
  • Assist with medication reconciliation and clarification of unsafe and unclear drug therapy orders.
  • Provide education to patients on medication use as needed.
  • Participate in education and training of healthcare professionals (e.g., physicians, nurses, pharmacy technicians, and students) related to medication use.
  • Demonstrate regulatory compliance through timely completion of nursing unit inspections.
  • Maintain professional competence with pharmacy department and state licensing requirements through staff development, independent study, and/or professional activities.
Qualifications
Education
  • Pharm. D. or BS in Pharmacy
Experience
  • Preferred: Hospital experience
Certifications
  • Required: Pharmacy license. AHA BLS, ACLS, PALS required if direct patient care responsibilities.
  • Preferred: Board Certification in Pharmacotherapy (BCPS), CGP, etc.
