Job summary

Valley Baptist Health System in Harlingen, TX seeks a licensed pharmacist to join our hospital pharmacy team. You will provide pharmaceutical care to patients, oversee medication therapy, and ensure proper documentation and regulatory compliance.

Responsibilities include reviewing orders, dispensing medications, ensuring IVs and dosing accuracy, and collaborating with physicians and nurses to optimize therapy. Hospital experience and active licensure are preferred; sign-on bonus may be offered.

Qualifications

  • Pharm.D. or BS in Pharmacy required.
  • Current pharmacy license required.
  • BLS/ACLS/PALS certifications required for direct patient care.
  • Hospital experience preferred.

Responsibilities

  • Review and monitor patient medication therapy for safety and efficacy.
  • Verify orders and perform medication reviews for interactions, allergies, and dosing.
  • Prepare, label, and dispense medications per physicians' prescriptions; ensure regulatory compliance.
  • Check high-risk IVs and conduct dose adjustments; manage IV to PO conversions as needed.
  • Ensure medications are available, properly procured, stored, and maintained.
  • Supervise preparation, maintenance, and dispensing processes and staff.
  • Maintain relationships with medical and nursing staff; provide drug information and regulatory updates.
  • Assist with medication reconciliation and patient education on medication usage.
  • Participate in staff education on medication use and regulatory compliance.

Job Summary This position may qualify for a sign‑on bonus. Provide pharmaceutical care services to the patients including, but not limited to oversight, monitoring, preparation, dispensing and proper documentation of patient medication therapy, as well as providing drug information and overseeing dispensing systems. Additionally, ensure compliance with Federal, State, and local laws and regulations and may advise physicians on medication utilization.

Responsibilities
  • Review and monitor patient medication therapy and orders written for safety and efficacy, considering specific patient conditions and health status; advise the patient care team and document interventions.
  • Verify orders and perform medication profile reviews, checking for duplications, drug‑drug and drug‑food interactions, allergies, drug appropriateness, dosing, and frequency.
  • Prepare, label, and dispense medications for patients in accordance with physicians' prescriptions; ensure compliance with Federal, State, and local laws and regulations pertaining to dispensing.
  • Check high‑risk IVs, renal dose‑adjusting medications, IV to PO conversions; initiate, modify, or discontinue therapy and mediate black‑box warning interventions.
  • Ensure appropriate medications are available, properly procured, prepared, stored, and maintained.
  • Supervise the preparation, maintenance, and dispensing process of drugs and supplies, including supervising technical personnel.
  • Maintain relationships with medical and nursing staff; provide timely information on pharmaceutical supplies, drug usage, compatibility, state and federal regulations regarding drug controls, and Joint Commission standards.
  • Provide collaborative practice management per facility‑approved protocols for anticoagulation, parenteral nutrition, vancomycin, and aminoglycoside consults.
  • Assist with medication reconciliation and clarification of unsafe and unclear drug therapy orders; provide patient education on medication usage.
  • Participate in the education and training of healthcare professionals (physicians, nurses, pharmacy technicians, and students) on medication use.
  • Demonstrate regulatory compliance by timely completion of nursing unit inspections as needed.
  • Maintain professional competence with the pharmacy department and state licensing requirements via staff development, independent study, and professional activities.
Qualifications & Certifications
  • Education: Pharm.D. or BS in Pharmacy.
  • Experience: Preferred hospital experience.
  • Certifications Required: Pharmacy license; AHA BLS, ACLS, PALS required if direct patient care responsibilities.
  • Preferred Certifications: Board Certification in Pharmacotherapy (BCPS), CGP, etc.
