Job summary

St. David's Round Rock Medical Center, part of St. David's HealthCare, is seeking a Pharmacist PRN to support pharmaceutical care.

You will interpret orders, provide therapy, compound and dispense medications, and monitor therapies to ensure safe, effective outcomes for patients at this acute-care facility. You will collaborate with healthcare teams, sustain formulary efficiency, and participate in clinical services including drug consults, PK, and anticoagulant monitoring, with a focus on safe,

Qualifications

• Pharmacist qualification and licensure per state requirements.
• Hospital experience and residency training preferred.
• ACPE IV sterile product certification preferred.
• Board certification in pharmacotherapy preferred.

Responsibilities

• Interpret medication orders and transcribe to computer systems.
• Provide medication therapy accurately.
• Compound and dispense pharmaceuticals.
• Monitor drug therapy for interactions and dosing accuracy.
• Suggest cost-effective therapeutic alternatives to staff.
• Maintain formulary and minimize non-formulary procurements.
• Assist with clinical services such as drug consults and anticoagulant monitoring.
• Complete notification reports for medication errors and adverse drug reactions.
